COVID-19 vaccination: Updated template survey questions

Find out how you can support the vaccination programme by undertaking research on public attitudes towards the vaccination. This resource includes template questions for the general population, as well as young people.

In response to COVID-19, the NHS has launched one of the largest public vaccination programmes ever.

To help local services and national policymakers understand how they can improve the vaccination programme, we have developed two sets of template questions you can use to engage your communities. 


  • One set of questions aim to help understand the general public's attitudes to and experience of the vaccination programme.
  • One set of questions aims to help you understand younger people's attitudes, especially those who have not yet been vaccinated.

Share your results

Local Healthwatch who are undertaking research can use their results to help local services better target communities and improve their vaccine service. 

Please also send a copy of any survey results and your final report to so we can use your evidence in the updates we regularly give to Government and NHS England policymakers.
