Download the annual report template to showcase the amazing work of Healthwatch during 2018-19.

Each year we provide a template to help you produce your annual report and meet your statutory requirements. This year's template is now ready for you to download from the Communications Centre.

The report template aims to help you tell a story of the impact you have made to people's experiences of health and social care services in your communities.

What's included?

We have worked with the network to develop a template that helps you showcase your impact. The template includes example text and guidance to demonstrate the type of content you should include, how much to write for each section, and different writing styles.

This year's template includes:

  • PowerPoint template (portrait)

  • InDesign template

  • Checklist on legal requirements

  • Photo library (soon to be updated new photography from Healthwatch events)

  • Graphics to help you demonstrate your work (soon to be added to the Communications Centre)

You will need to have your annual report submitted by Sunday 30 June.
